Heavy Bread Questions

Last nite I made potato bread. I used my bread machine. I bought more yeast yesterday because when I made bread the other day it was quite heavy (didn't rise enough) and upon checking my bottle of yeast it was well past it's "use by" date. My bread flour is a year old but it looked and smelled fine so I used it. My bread came out short and heavy with a small bit of doughy-ness @ the center. I called my mom and asked what I did wrong again! She didn't think the bread flour should have made the difference but "hey who knows?" she said. She suggested tossing out my flour, buying new and trying again. She also said she keeps her bread flour in the freezer. So I am hunting for some suggestions from all you homesteadin' bread bakers! Do I need to freeze my flour? Does flour "expire"? Do I need to store my yeast in the fridge?
